High Voltage - Fused Cutouts and Arresters

When is it best to use a combination fused cutout/arrester vs. separate components? Also, do you always need both a fused cutout and an arrester or just one or the other?

As far as a combination type, I have never seen one and we do not use them on our system. If they are available, someone must be purchasing them so my observations are not totally valid.

The Code doesn't require lightning arresters. We install a set every 1200 ft, every open or deadend location, and at every equipment location. The location of cutouts should be planned by an engineer who can limit the load on taps at their load end instead of everywhere the conductors change sizes. The art of overcurrent protection on large medium voltage campuses can be a daunting task that is best left to a qualified engineer as stated in 240.100(A). :D
