You should have answered that you are certifiable.
There may be some local rules for the particular hospital in this regard. Maybe you can reach out to this person and ask what he/she meant by such a comment.

I would guess if it means anything at all, your state may certify certain contractors that make an application and meet some experience requirements.
For instance in Mass a contractor can only bid public jobs valued at more that 100K if they are so called prequalified. Here the prequal requirement would not apply to a hospital unless it was owned by the state.
