You would/could use "tap and run" molds for all the rods.

Get out the cadweld catalog and figure out what molds you need.
It's pretty easy. You should be able to figure this out.
Either molds with the wire passing straight through or molds with one wire and tee molds for the ring.
 
Exothermic welding (Cad Weld is a brand name) is the best way to do it, but you can also loop an unbroken length of wire through the ground clamp at each rod then at the return end add the final clamp above the starting clamp, that way only one wire is in each clamp maintaining your UL listing.
 
It is better to make the 3/0 a continous run and use a pass thru mold. Cad weld now makes a throw away mold that has the powder premeasured and sealed which uses an electric igniter with a long cord, much safer and cost about the same as the old style. Once you fire it, you just break the mold away.

It must be some old school method that has just carried over to now.
 
The delta ground rod configuration is for lightning protection. I suspect it intended to spread out the lightning impulse over a wider area.
Its one of those engineering specs that keeps getting copied over and over.

No, it's more like a single 3/0 conductor laid across the top of the rod, in a T shape, and welded on that way.
