How to tap load side of 400A breaker for a Type 1 or Type 2 SPD?

What wiring methods have you used to connect the SPD spagetti-size wires to the breaker terminals or feeder conductors?

I'm scratching my head to understand the best (NEC compliant) method.

Most methods I have seen just appear to have had the SPD wires inserted into the loosened termination next to the conductor...
 
Although I have seen many connected as you described, IMO, it is a NEC violation unless the lug is properly rated for such a connection.
I may be incorrect, but my memory is that the Type 2SPDs I have seen have a manufacturer OCP requirement far less than 400 amps.
