IMO, those apply after the fused disconnect. If the disconnect wasn't fused they would apply all the way from the panel to the unfused disconnect and load.

They are in subpart V titled Motor Feeder Short-Circuit and Ground-Fault Protection, content in those sections includes motor(s) only as well as motor(s) and other loads. Feeder supplying a single motor essentially gives you same conductor and overcurrent device as for the branch circuit to that motor. You can increase things beyond code minimum here if you want, but you have to consider both conductor and overcurrent protection if you do. This is covered by (B). Kind of means if I want to run a 200 amp feeder even though I currently only have a 20 amp motor load I still can if I put in 200A OCPD and conductor on the feeder.
