Installing second external disconnect for mobile home

The utility company installed a pole with a meter and 200 Amp panel with disconnect. It is 250 feet from the mobile home site. A 4-wire service connection needs to be made from the disconnect panel by the meter to the main panel in the mobile home. However, if I understand the NEC code correctly, there needs to be an external disconnect within 30 feet in eye sight from the mobile home. So I need to install a second disconnect (call it disconnect B) within 30 feet of the mobile home which will come between the main disconnect (call it disconnect A) by the meter and the main panel in the mobile home. My questions are the following:
1. Is my understanding correct that I need to install Disconnect B within 30 feet of the mobile home (i.e., I cannot go directly from Disconnect A 250 feet to the main panel in the mobile home?
2. Do I run 4-wires from Disconnect A to Disconnect B?
3. Neutral and ground bars are connected in Disconnect A. Do I also connect them in Disconnect B, or would they be kept isolated in Disconnect B.
4. I assume in all cases I will need a grounding rod for Disconnect B.

They don't like DIY questions on here. You might be allowed the actual code questions portion of the post. What kind of engineer you are might affect the answers you get if any.
I am an electrical engineer, but I specialize in chip design, so not so familiar with NEC code and so forth. Sorry, I'll post my question somewhere else.
