Insulated equipment grounding wire?

When I looked for identification of Equipment Grounding conductors, NEC(250.119) says that "unless required elsewhere in this Code, equipment grounding conductors shall be permitted to be bare, covered, or insulated.

But in real field, we only used insulated equipment grounding conductors except electrode grounding conductors which we used as soft drawn bare copper.

So do you know any regulation in the code prohibit the use of bare equipment grounding conductors? Thank you for your response in ahead.

There are articles that require an insulated conductor, so in a sense, yes there are prohibitions to using a bare conductor, see 517.19(B)(2) for one location off the top of my head.
