Is this saying if I have two dedicated convenience receptacles fed by two different branch circuit hots with a common neutral(12/3 with ground), in the same box/coverplate ( appears to be just a plain vanilla 110V Duplex),that the two circuit hots must be on a 2 pole breaker at the panel?

Kind of.

It does not have to be common neutral circuit, it could be two 2 wire circuits and you could use approved breaker handle ties in place of a 3 pole common trip breaker.
 
So we are talking of two different scenarios,,,,,,

A split wired duplex receptacle with two independant circuits or a shared neutral,in either case they would have to be on a two pole or two single pole w/handle ties?

Thats the way I read it. Art 210.4B also supports that.

Yes, between the two sections both multiwire and two wire circuits are covered.

The unqualified DIYs have been protected.:roll:
