My question regarding FPN No. 2 in article 215.2 is determining if the conductor between a main panel and and sub-panel would be considered a feeder or a branch circuit. I think that the main disconnect just before the main panel is considered service equipment and the conductor between them is the feeder. But what about conductors between panels, feeder or branch circuit?

Take a look in the Definitions of your code book. In the 2005 NEC the definition of Branch Circuit is on page 70-26 at the bottom right hand side. It states: "Branch Circuit. The circuit conductors between the final overcurrent device protecting the circuit and the outlet(s)."
Also take a look at Circuit Breaker on the next page (this is the overcurrent device) and the definition of Outlet.
