To take a 3 phase 3 wire 200A 208 V feeder and feed a transformer and get out a 3 phase 200A 4 wire 208V circuit? Never seen it done but an curious if possible?

Sure. And it is sometimes done for isolation purposes or if you don't have a neutral but need to create one.
