Isolating Neutral and Ground

How do you properly isolate (float) a neutral from the panel and ground if the neutral bar and ground bar is permanently bonded? I've seen a few ways but don't know if they were the proper way.

You change the panel because if you look the label it will be listed as service equipment only.
 
I guess what I saw were small used small panels that someone decided to use for this purpose. I know there are "service equip. only" and "suitable for service equip" panels. I suppose I should have asked if it's allowed.
 
I probably would use the existing bar/s for equipment grounds and add an isolated bar for the neutral.

Thanks Dennis, One thing that I heard someone say they had done made me wonder about this. They said they wire nutted the neutrals together and landed the egc on the ground bar. That just didn't sound right so I thought I would ask if there was a proper way.
