I was at the Supply House the other day and saw the this breaker on the counter.

It is a bolt on, 20A 120V with a fuse holder.

I have not seen one of these before, how about you?

Almost every manufacturer had these back in the late 50's (the Square D one was a QF?).
IIRC, they used a special fuse and were limited to about 5kAIC. I believe they were all gone by the wayside by the time of the UL SCCR changes in the early 80's.
There is still language in the NEC about integral 30A switches and fuses, which might apply.
