Double wires on breakers and neutral bar although the 2 wires under one lug for the neutral may have been compliant when it was installed. White wire used as a hot may not be taped.
 
Other than the obvious violations....
Is it me or are most of those #12 terminated on 30A breakers?
Can't see very clearly, but I can swear it's a 3 there on most breakers(least the double poles)
 
I think its an ITE/Siemens. Looks like several types of breakers, SqD HOM, CH-BR mabe, GE?
Are they all listed to be installed in that type of panel?
Can't tell if the bonding bar at the bottom is installed, connecting the two ground/neutral bars.
Assuming this is the main means of disconnt is it bonded? Dont see a green screw or anything obvious.
 
Is there a neutral feeding the panel? Don't know if I'm not seeing or it or if it's not there.

Also, can somebody explain to me the 4-pole main breaker?

On a picky violation note, my guess is that panel is rated for that many double breakers to go with all those full size breakers.

I don't think two grounded conductors under one terminal has ever been compliant. It was part of the listing instructions. The only reason it is now in the code is because installers were not reading the label on the panel.

Its a 2 pole breaker. The manufacturer designed a 200A main by basically putting (2) 100A breakers in parallel.

In general, breakers can't be put in parallel, but the manufacturer can if they get the proper listings (for lack of a better way to phrase it).

Look to the right side of the Main Breaker and you'll see a white taped full sized current carrying conductor landing on the top lug of the right hand neutral bus.
