Jobbing/site specific paperwork

I'm looking to see if anyone has any type of job tickets or return trip tickets they can share. As our shop grows, we strive to keep the projects efficient. Having my techs fill out paperwork has become an issue - Cardboard and market doesn't cut it.

I use these from Deluxe Business Forms (formerly NEBS)

They have several ready-made business forms 2 and 3 part with info on the back cardboard copy. They also have metal clip boards

Ours are basically like what is listed above, but in landscape format; I highly recommend that. I like having longer lines to fill out instead of using multiple lines for one item if it's a longer description.
