Anyone have a good JSA they can share? Not all jobs are the same and the ones we currently use are prefilled but dont do much good when the SOW is always different. We are looking to make JSAs prefill by each Lead onsite.

Job safety analysis
Statement of work
Guessing
You could use a task assignment format and tie in a Hazard analysis and put that in you OSHA job site safety folder.
The task assignment will have a section for the work, personal, any tools, area of work and associated Hazords. A sub category can be added for personal safety training so a lead can be the competent individual for the task.
These can be summited for review and have GC sign off on work. These will tie into there scope. Helps account for personal, and any effect on other trades and or owner.
The GC safety officer can attend task assignment briefing and sign off. Some places the GC has to get owner to sign off as well.
I dis like paperwork to add an receptacle or check a light. A past job when I and two other were simply walking through an area we were stoped and all paper worked checked. The GC said thank you for have the paper work in order and doing so within guidelines laid out in the task assignment. The personnel that stoped us was owner,I mean the owner his safety guy, GC owner and there superintendent and GC safety. Book opened up with all signatures with approval. Equipment listed in paper work checked. Had to give verbal task assignment including any all Hazords associated with task. When I say equipment I mean every tool was checked. Owner safety guy even checked to make sure boot laces were tied.

My guys were freaking out being lead I told them to not speak unless spoken to be direct and accurate.
What we found out was they were on the way around checking plant. 28 men lost there jobs that day and was escorted of the property by security.
My crew was not one of them.
Some days it pays to do the paperwork.

I tried loading the Word version but it would only allow a pdf. But here's an example. It's outdated, but serves the purpose.

JSAs we’re just starting when retired. Up till then it had been tailgate talks. JSA concept seems better and for many companies they could be boiler plate modified for particulars
