Do you guys ever use the 3-piece plugs with a screw/bar attachment like the one below?
They're more expensive but easier to attach in certain limited clearance situations.

No, but I have used the two piece ones with a screw and nut in the center for large KOs. Other than that the snap in ones work fine. I've never had a problem with getting them into tight places. Lay a screwdriver over it and and smack the shaft with your linesmans as close to it as you can get.

If you have so little 'clearance space' in a box that you can't get a push-in blunder plug to work, the box is overfilled.
 
No. Something to work loose and fall into the panel.

Does not happen. Had problems with the one piece ones falling out though industrial plants. It’s actually a 4 piece. There is the grey outer cover then a piece of foam rubber gasket/seal that makes it NEMA 4 under that. Then a conduit reducing washer piece that goes on the inside and finally a wing nut that squeezes it all together. Works great. Biggest brand name is Hoffman so there’s most of your high price with the name,
 
The one piece ones are also a pain to splay out all the wings after you snap it in. Some people skip that step.

I don't think you're supposed to need to, but the prongs are not springy enough for a snug fit, so spreading two or three is par.

They're also tough to snap into many KOs, but I've found prying them in with Channellocks to be easier than banging them in.
