Should all LV cables from utility transformers (3 Phase 4 wire 4160/480V) to service entrance equipment installed in PVC conduits be de-rated?

I don?t understand the question. Derated for what condition? If you have more than three current-carrying conductors in a single conduit, then you derate for that condition. If the ambient is more than 30C, you derate for that condition. Just being in PVC will not call for a derating, nor will just being a service entrance conductor.
