would a 1200 amp disconnect that has meter packs
with disconnects that are connected to the 1200
disconnect by bus bars, located inside an electrical room, about 12ft long be considered
large equipment and have to comply with entrance
requirements.

the 1200 amp breaker is fed from a transformer,
and the meter packs are bolted to both sides of the 1200 amp breaker

Please read 110.26 (C) carefully to understand that this section does not require an overcurrent device rated 1200 or more present in order for the two doors to be mandated.
"For equipment rated 1200 amperes or more that contain overcurrent devices, etc."
The 1200 ampere rating of the meter center, with a tap box, or with a main OCD, both fall under 110.26 (C).

Please note also that the 2005 version would not require the equipment to be more than 6 feet wide. But the equipment you describe would fall under the 110.26(C)(2) rules for "large equipment," even under the 2002 NEC.

thanks for the responses, I failed this job for no second door as per 110.26 (C) and the engineer does not agree with me, just wanted some other opinions.

What are the dimensions of the room and how much working space is provided in front of the equipment? Is it less than 150V to ground?

(FYI)Check exceptions 110.26(C)(2) (a) & (b)

You may not necessarily have to have the second exit door.
