In angle pull section 314.28 in using a pull box or a conduit body with a opening for a 2" conduit the space between should be 6 times + 2 or 14". If this is true than all lb conduit bodies are in violation.
example, in a 200 amp service using 3-2/0 wire in a 2" lb conduit body with 2" pvc is not approved.
 
A conduit body maybe listed for use in physical sizes that do not comply with 314.28(A)(1) and (A)(2). This is permitted by 314.28(A)(3) as George mentioned. This is because as part of it's listing the manufacturer has determined that the conductors can be installed without the usual 6X and 8X dimensions without damaging the conductors.
 
But look at the wire sizes that are marked on the conduit body...they are often much less than what you are permitted to install in the raceway.

A question I have is, what do we do about it? Upsize the race for the LB, and return to the smaller one afterwards?

Yes, you can use a conduit body that is larger than the raceway and is of suitable size for the conductors being installed, or there are mogul conduit bodies available. I have also seen a section of square duct used in place of a conduit body for this purpose.

Don brings up a good point. Many times in the field I have seen guys install C condulets in runs of large conduit to satisfy the 360 degree rule. A 3" EMT is permitted to have 5-500Kcmil THHN conductors in it. I'd like to see someone pull that in and out of a 3" C condulet. It is important when using condulets to make sure that they are sized properly for the conductors that will be pulled into the conduit otherwise they are next to useless.

I have often used a section of square duct 6"x 6"x 48" in a 4" raceway as a pull point.

Assuming a straight through pull you can keep the square duct as narrow as the lock nuts will allow but you still get 48" to feed the wires into.

We will also use square duct as oversize LBs.
