What is the minimum mounting heig [*] ht in a general office area?

Standard door height is 6'-8". Minimum ceiling height is usually 7'-0". Some commercial doors are 7'-0" or even 8'-0", so personally I wouldn't go any lower than 7'. There are more and more 6'-8" people. Most of this world is sized for the 5'-2" female to the 6'3" male. Anything beyond that is optional. That's my story and I'm sticking to it. :cool:

A. No, but where the American Disabilities Act is applicable, Section 4.4.1 states that ?no object mounted between 27 in. and 80 in. above finished floor is permitted to protrude more than 4 in. from the wall.?

If a light fixture is in an egress path, it must be above the minimum height for the egress path. You should look this up but I think 7'-6" may be the minimum head clearance for an egress path. I would use this as a minimum for all of your fixtures. You can make an exeption for wall sconces if you follow the ADA rules awwt mentioned.

Not quite. A means of egress is made up of three components: The exit access, the exit and the exit discharge. If you are not in an exit (enclosed stairway) or you are not in the exit discharge (outside) you are in the exit access (every where else). As stated before, the area between 27" and 80" may not have a protrusion more than 4". The intent being that a visually impaired individual cannot use a cane to identify hazards.
