I'm installing 8' flourescent fixtures in a retail store without lenses onthe fixtures and another electrician said that the lights needed lenses for safety incase the bulbs were to fall out. also on ballast replacement we always install the quick disconnect plugs on the replacements. do we have to install them on new installations. Thats for any help.
 
No and no as for requirement, or yes and yes if you want to limit liabilty and make it easy for yourself down the line when you want to change them - if you end up being that guy...
 
it seems we are always required to protect the bulbs - either sleeves, lenses, or metal cages. However, I don't think its NEC (I couldn't find it in the code anyway?)

No, it is effective 1/1/08, if your under the 2005, the requirement is in the 2005 NEC to become effective 1/1/08.
 
Open lamps are allowed and are common in many retail stores.

In commercial locations over food, in kitchens etc have to be shielded per health codes or FDA.
