Does NEC 110.26 apply to lighting controls in 6X6 inchelectrical boxes on walls? What about the same type controls mounted overheadmounted from conduits from 12X12 junction boxes?

Taken literally there is nothing that 110.26 doesn't apply to. Even a code required kitchen counter top receptacle is in violation.

The NEC Handbook commentary, which is only an opinion, shows us how dysfunctional this section is.

Minimum working clearances are not required if the
equipment is such that it is not likely to require examination,
adjustment, servicing, or maintenance while energized.
However, access and working space are still required by the
opening paragraph of 110.26.

This section specifies the required working space for
such equipment as panelboards, switches, circuit breakers,
controllers, and controls on heating and air-conditioning
equipment. The intent is to provide enough space for person-
nel to perform any of the operations listed without jeopar-
dizing worker safety. The word examination, as used in
110.26(A), includes such tasks as checking for the presence
of voltage using a portable voltmeter.
