Is a recessed fixture considered a lighting outlet? Is a lighting outlet for example a round box that a light fixture is attached to?
If so, has it always been this way?

imo, the fixture is a light fixture but the lighting outlet is the point where the wire attaches to the fixture. It is usually a box.

Yes and yes. As far as always this is from the 1965 NEC:

*Lighting Outlet:
An outlet intended for the direct connection of a lampholder, a lighting fixture or a pendant cord terminating in a lampholder.

*Outlet:
A point on the wiring system at which current is taken to supply utilization equipment.
