What is the minimum requirement in the code on Lightning protection?

Is there a minimum height of the building wer lightning protection should be installed?

How do i determine if lightning protection is required? or not required

Lightning protection, is not an NEC issue, there are a few references to lightning in the NEC, see this footnote in Art 250
FPN No. 1: See 250.60 for use of air terminals. For further information, see NFPA 780-1997, Standard for the Installation of Lightning Protection Systems, which contains detailed information on grounding, bonding, and spacing from lightning protection systems.
Lightning protection is an optional installed system. Its the risk versus the cost.
