How do I know if I have to protect a building from lightning?

Is there a min or max height?

You don't "have" to unless its sepcified by the engineer/arch/designer/etc.. Once its been determined that lightning protection is required, they must also tell you what reference it must conform to. (NFPA 780, etc...)

Lightning protection can also be TVSS or surge arrestor installation.

There is no NEC requirement, strictly a design issue.
