To my knowledge, they are not mandatory, there may be a few jurisdictions here and there that may make them mandatory.
Most times they are specified on the job by either the engineer or the owner.

That is what I thought but could not verify. I looked in NFPA 101 and could find no req'mt there.

I can't speak for Baltimore, but Florida mandates lightning protection for a few specific occupancy types as outline in the building code, which references the NEC and NFPA 780 for methods of compliance.
It is also possible for some other agency or "AHJ" to mandate the installation of an LPS. An example would be an insurance underwriter covering a warehouse or other structure where combustible storage is located.
