I know it is industry standard to have the Line on Top and the load on the bottom of a fused switch or ECP, but is there any code that requires it?

240.40 will require the line connections to be on the top of most, if not all, fused disconnects. I don't think there is anything to require that for teh enclosed circuit breaker, unless you have a breaker than is marked line and load on it.

Knife switches (those that are on control panels in monster movies) are supposed to be mounted with the handle of the switch hanging DOWN when the power is off. You raise the handle and press into the live connections. Reason-- Fails to the OFF position.
IIRC-- that's what the knives in a basic switch box do.
So-- the power comes in at the top, through the switch, and out the bottom.
