Line diagram updates for NEC compliance.

I'm working on obtaining a permit to upgrade my electrical panel from 200A service to 400A service and my permit request was kicked back for the following reasons


1. Refer to one-line- diagram, Provide grounding electrode system. Please show compliance. (2011 NEC 250.32)

2. Refer to one-line-diagram, subpanel notated on one line diagram (125AMP), provide information as to the
location of subpanel, is this for an additional structure, provide clarification. (2012 IRC R106.1.1)

item number 2 seems straight forward, I'm guessing I just need to indicate that the sub panel is located on the interior of my detached garage. (and probably needs to show the grounding electrode system as well)

Item number 1 seems straight forward as well, But I'm struggling to find adequate documentation on what the grounding electrode system looks like on a diagram such as this. Can anyone point me towards an example diagram I can use for inspiration?
