Often my understanding of the code is greatly improved when a colleague explains the type of failure or accident that article is addressing. Is there such a listing?

Every statement in the NEC was put there by a committee. Changes take place every three years. Anyone can offer suggestions on new requirements or on rewording existing requirements. The committees review the suggestions, make their decisions, and reissue the code. They document the suggestions, their comments, and their decisions in such documents as "Reports on Proposals" and "Reports on Comments." The NFPA maintains copies of these ROPs and ROCs and other related documents. They are for sale, I believe. But there is no easy way to sory through all of that information (untold shelves of documents) to find the basis for the present version of any single code requirement. And I no of nobody who has done so, and who has prepared a simple list. Sorry.
