Location of Group Lock-Box's In An Electrical Energy Isolation Program

I have been on Projects where the use of Group Lock-Boxes have been lost, stolen, moved around, broken into or locks cut off. In one case our safety team found a cut lock on the main energy isolation device removed for emergency reasons by operations with all locks intact on the lock-box. In another case, a fellow needed an ammo box and walked away with the box that was in use.
Questions:
1) Where should lock-boxes be located? In my opinion Group Lock- Boxes should be located near the main energy isolation device to enable lock owners to visually confirm the proper position of the isolated main energy device. Such verification is required before each days work.
2) For security reasons should lock-boxes be mounted in a fixed position near the energy isolation device.
I have contacted OHSA on this issue and they referred me to an ANSI Consenes Standard and an OSHA reference. Neither of these documents address my questions.
Any comments from electrical professionals would be very much appreciaed.
