Or in the case of the main service, safety grounds.

Safety grounds still need to be remove, so I put my locks on the utility switch until all is clear, I would like to know how the utility reconnected if everything was locked out by the contractor?

The proceedure with the transmission utility up here does not include locks. They simply tag everything. We are more then welcome to add our own tags, but in many cases the gang-switches don't even have provisions to be locked open, even if we wanted to hang our own locks.

I absolutely agree that safety grounds should have been installed. While it might not have eliminated injury, the guy probably wouldn't be clinging to life.
