Lock out means for Transformer primary conductors

Is it stated and if so, where in the code, the need for a lock-out means for the primary feed to a transformer?

A transformer 480 to 208-120 wye was recently installed in an electrical room in a department store to feed a lighting panelboard with its own main protection. However there is no disconnecting means located in this room for the primary windings for the transformer. If the transformer supply overcurrent disconnecting means is located out-of-sight is a lock-out means integral to the switch neccessary (for worker protection)?
