Lock-Out Tag-Out

Is it the OEM's responsibility to place lock-out tag-out labels on the equipment, or is it the end users responsibility to add this labeling?
 
What labeling are you asking about? Typically labeling is permanently installed on the equipment, and tags are required for the locks. I am not aware of any required lockout labeling.
 
I'm not sure about the exact specifications--but I would, as the posters above suggest--do both if at ALL possible.

If it were up to me, I would try to utilize
"defensive engineering" so as to mitigate
any liability that you can.
It would probably be best to devise your
label, and if the end consumer deems it
necessary they can add their labels and
inform the operators of your equipment
on the proper isolation procedure. Your
label protects you (designer) to a certain
extent if something catastrophic were to
bappen.
In these days liability problems come up
more frequently ( than in my opinion should )
So try to look at this as legal preventive
maintenance.

I think it is convenient if you provide disconnecting means with locking provisions, and that locking and tagging is the responsibility of those working on it. This lockable disconnect possibly is required to meet listing requirements if you are building listed equipment - IDK about this one for certain, or it is possible you could even specify in instructions a disconnecting means be field installed and that it must meet NEC requirements and that could get you out of everything you ask about. Seems to be a lot of equipment that we must provide our own disconnect as well as a lot of equipment that has a disconnect already included out there.
