According to the 2008 NEC, are the portable devices no longer acceptable for general lockout in an industrial / commercial environment acceptable? Please refer to attached picture.

If not acceptable, how will one lockout a circuit breaker?

You can still use the portable devices to comply with OSHA loto requirements for existing installations.

What has changed is now the NEC does not recognize those as a way of locking the disconnecting means open.

All this means is now you must order the factory lock out kit for the breaker at the time of construction and fasten it to the breaker.
 
OSHA tag-out rules can also be used without a lockout if the means for locking out is not available. This tagout rule typically requires two places that the circuit is disconnected and a bunch of stuff. I can provide the OSHA rule # if anyone wants it.

How this fits in with the NEC rules, I am not thinking of that right now.
 
As far as OSHA yes tag out can be used nothing, has changed with OSHA.

But with the NEC if you decide to forgo putting a disconnecting means at an appliance with the reason being that you can lock the breaker open you will also have to provide and install a permanent place for lock to placed on.
