Lockout/Tagout

Does anyone know what year OSHA started the Lockout/Tagout program.
I have two different dates, one is 1991 and the other is 1989. I went to training classes in the early 90's on Lockout/Tagout but don't remember the exact year.

Lock Out, Tag out is slightly changed now:
A. Determine all possible sources of supply
B. Properly interrupt the load and open disconnect(s)
C. Visual verification of blades
D. Lockout/Tagout
E. Verify absence of voltage with meter, with the read, verify , read method. Simply put,, you have to go to a verifible souce to properly read the meter. Then go to the circuit to verify absense of voltage, then take the meter back to the verifible souce to make sure your meter is operating properly.
F. Release stored energy and apply ground straps.
G. WHEW......

On E. My method is to confirm voltage at the disconnect before opening disconnect. I have seen far too many times when the wrong disconnect was locked out; which leaves it possible for the correct one to be turned on after starting work. It is not always possible to see which disconnect is the correct one. When possible I have meter on the disconnect when it is opened and then turn it back on to confirm it is correct disconnect...then turn it off and again verify absense of voltage.
