In most situations, yes.
There could be some situations where the equipment supplied did not need to be grounded or the voltages were low enough to not require grounding.
Low voltage lighting is one case that comes to mind.
 
250.118 does not list PVC as an accepted EGC. So that means you must supply an accepted EGC. For most cases, that's pulling in a green wire.
 
Yes , 250.120 I belive.
It really does not matter if the conduit is underground or not. For the most part Service entrance is the only time there is not a grounding conductor.
 
i see. i think 250. 118 allows for some raceways to be used as egc depending on the ampacity of the breaker when it comes to branch circuits, but i just wanted to be sure about pvc and see if there was a specific reference. thank you.

Yes you can use EMT, IMC, and Rigid conduit as your EGC. Now for PVC conduit, It is not conductive so a separate EGC would be required for all sizes of PVC conduit.
