What is low , medium, and high voltage?

Please reference code book.
I'm looking for the numeric scale for each.

There is no standard definition, in the code or otherwise. If you're not sure about someone's usage, ask them. On some projects 'high voltage' might mean anything at 120 or more and on others it might mean hundreds of killivolts. 'Medium voltage' usually refers to distribution voltages in between typical premises wiring and long distance transmission voltages, but precise numbers will still depend who you're talking to.

The NEC makes significant and widespread distinctions at 1000V (used to be 600V). Other distinctions, usually at lower voltages, depend on specific applications.
 
IEEE has defined five different voltage classes for power distribution equipment.

Low 600V or less
Medium 601-69KV
High 70-230KV
Extra High 230-800KV
Ultra High >800KV
