I’m looking for the code that states a pull box is required after every two 90 degree turns. Also, what qualifies as a pull box (lb, box, etc)
 
Most raceway requires a box after four 90's.
You will find that in the particular raceway article that you are using in the section that ends in .26. For example EMT is 356.26.
356.26 Bends — Number in One Run. There shall not be more than the equivalent of four quarter bends (360 degrees total) between pull points, for example, conduit bodies and boxes.
Does your cable type require raceway?
 
This is for low voltage cat 6 cables. The raceway is being built currently but does not have enough pull boxes
You will have to determine what the Cat 6 is being used for. Is it for Remote-Control, Signaling, and Power-Limited Circuits such as Article 725 or Communication Circuits Article 800, Network-Powered Broadband Communications Systems Article 830, Premises-Powered Broadband Communications Systems Article 840 or one of the others. Then see if it refers you to one of the conduit articles which would impose the bend limit.

Most of the time in these situations, it is a Contractual requirement or a cable manufacturer limitation for performance
