In my opinion, per 680.43(B)(1) includes luminaires mounted above the unit including on the walls and within 5'of the inside walls of the spa. If the fixutre is below 12' (I assume it is), it would have to have GFCI protection, and the fixture would have to meet the requirements of 680.43(B)(1)(c)(2).
I don't think a hydromassage bath tub would have the same requirements. The difference between the two seems to be that a hydromassage tub is designed to have the water drained each use. A hot tub is not designed to have the water drained each use.
