Luminaries installed in a shower & over a bath tub

Does the NEC require Luminaries installed in showers and above bathtubs to be protected by a GFCI? This is an existing condominium with a separate circuit for the lighting.:?
 
No. The only time it would be required is if the manufacturers listing of the fixture required it.
 
Normally a bath fan requires a gfci if the fan is installed over a shower however that is not an NEC rule but the manufacturers installation instructions

Yes, and, most times a dedicated circuit is not pulled strictly for the bath fan unless its a Fan,Light, Heat combo type.

The fan is usually on the same circuit as all of the lighting in the rest of the bathroom, so,,,,
