Main breaker backfed: required tie-down

Y'know, when I look closer I can see some notches on the edges of that breaker: perhaps it's supposed to be done this way on these breakers. There's some paralax that makes it look like it goes through one of the breakers, to me.

Yes, there are channels molded into the breaker bodies, and a hole in the pan flange.
 
Wow: I'm sort of speechless at this, am I missing something? Obviously it's been this way a long time... has anyone seen it done this way???

If you are referring to the screw in center of double pole it is mandatory for a backfeed main to be secured to prevent possible removal while energized, mfg makes it to be secured in this fashion. Based on image and labeling next to it, it would appear to be a backfeed main breaker.
 
Yes this is a common, code compliant method of securing a back-fed circuit breaker.
