Main breaker/disconnect outside and disconnect in panel

I have a customer who's main panel is 20 feet inside of the house. We are upgrading the service to 200 amps. We have a meter base with a 200 amp Breaker installed. Can I use a 200 amp main breaker panel if I separate the neutral and grounds. I have had discussions about not having two disconnecting means but not sure if there is a specific code spelling this out.
 
As long as the panel you are using is designed so neutral and ground can be separated you're okay. Most would be fine, of course, as long as it's not a meter/main combo.
