Main Breaker in Panel

Is it a 2008 code to must have a main breaker in a distribution panel ? I have a Disconnect with a proper lock out ( the bolted to breaker ) in a power plant and then would like to set a panel on the first floor of the building. Am I required to have a main CB. in the panel or will main lugs serve just as well what is the code requirement for this ? The old days you did not have to have a main CB as long as it was under six throws or if the disconnect serving the panel had a lock off means just checking to see if this is still okay. Thanks

hoe are you reading this sounds like I can lock out in basement disconnect then go to main lugs of panel board.
 
Panels in the same building as the power source for that panel do not require a local means of disconnect. The only requirement is that that have overcurrent protection at or below the rating of the panel.
 
A service panel may have a main breaker or as many as six breakers to disconnect the service. A sub panel or remote panel in the same building does not need a main breaker nor does the six disconnect rule apply. Thus you can have a 60 circuits panel with no main breaker.
