Main Breaker

Do I need to install a main breaker in small
apartment sub-panels that have about 12 or 13
branch circuits and is being feed from a meter stack
with the main disconnecting means located in the meter stack?

Main breaker not required , but occupant access to the disconnect must meet this:

240.24(B) Occupancy. Each occupant shall have ready access to
all overcurrent devices protecting the conductors supplying
that occupancy, unless otherwise permitted in 240.24(B)(1)
and (B)(2).

(1) Service and Feeder Overcurrent Devices. Where
electric service and electrical maintenance are provided by
the building management and where these are under continuous
building management supervision, the service overcurrent
devices and feeder overcurrent devices supplying
more than one occupancy shall be permitted to be accessible
only to authorized management personnel in the following:

(1) Multiple-occupancy buildings
(2) Guest rooms or guest suites

Look at this way, why are you asking the question? Is this asn existing installation installation, a price sensitive proposal where you have competion or?
If a main wasn't required if the additional cost for a main is a non issue by including as main CB would prove the tenent with a no brsiner should it be required to shut off the power in as hurry. If you have to go to another location to shut off a main in a hurry they better know where the main is located.

If a main wasn't required if the additional cost for a main is a non issue by including as main CB would prove the tenent with a no brsiner should it be required to shut off the power in as hurry. If you have to go to another location to shut off a main in a hurry they better know where the main is located.
My house is like that. I HATE the guy that decided to do it that way...

Yes, for me if being competitive where you price is based upon the minimum requirements is one thing in prefer to win the bid. If a modist price increase is not objectionable tto the client I always look for a user friendly installation that makes sense to the owner. The addition cost for a panel with a main adds a small percentage to the overall cost of a job which often times is basically insignificant.
