Main Disconnect

I am a Home Inspector in Missouri. Is it code that the main disconnect must be within 5 feet of the garage door if the panel is mounted in the garage, or just a requirement of the city I live in? I have seen several in different jurisdictions that are not located within 5 feet of the door and the main disconnect is mounted inside the panel. A new construction I saw today they had to put the main outside the garage at the meter loop to meet this requirement?

The NEC does not require the main disconnecting means to be located in any particular location, it has a few places where it can not be located. This sounds like a local requirement that was pushed by the firemen for a standard location. Some places require the disconnecting means to be located on the exterior of a building.
