Main Lug or Main Breaker Panels?

Hello,
I have allways installed sub-panels with a Main Breaker in it. I see a lot of main lug panels out there (as sub panels)
Are there any limitations where you cannot use the main lug panels? Distance of run a factor? Can a Main Lug be installed anywhere? I know that a main breaker is needed after a transformer, or Main panel.

There is no place in the NEC that requires the use of a main breaker panel.

Sometimes using a main breaker panel is an easy way to comply with other requirements such as transformer overcurrent protection or a disconnecting means for a separate building or structure but both of those examples could be accomplished other ways leaving the option open of a main lug only panel.

My rule of thumb on this is that I always use main breakers for subpanels in detached buildings. If the subpanel is in the same building that it's being fed from, a MLO panel is fine.
