please could you help me:

to connect 4 wires on male plug with 5 prongs,
4 prongs are in a circle with markings X Y Z W ,
the 5th prong is in the middle and it's ground,

i am wondering what those xyzw mean and which wires go where?
i would connect white wire (neutral) to W,
what about x y z?

those xyzw are markings on a male plug which is connected to the machine,
that machine is 20 amps, 208 volts, 3 phase,
i would use #12 wire,

how about receptacle ?
i would connect white to neutral on the panel,
what about other 3 wires i know they go on breaker on the panel
but i don't know is there an order that they are connected and how do i know,

x y z are your 3 phase conductors. W as you said is the white or grounded conductor and the green ground-- well you know where that goes. :grin: No order-- if the machine has a specific rotation you may have to switch the places of 2 phases

equipment is very critical and i do worry about the rotation,
i am scared to start it,

three pole breaker is still OFF,
i will go again to check everything before
i start it,

If it's truly critical, you may be able to test both the motor and the conductors before you energize them.

UEI and Greenlee both have them.

You may be able to 'bump' the motor by turning the power on and off as quick as possible. This can turn the motor slightly, but not enough to cause damage.

I had one of these in my hands this weeks, it was sudo mis-labeled in my mind too, or you could argue other, the W,X,Y,Z where all the same color terminal screw. OK, I had moment ...

The color was implied by looking back through the clear plastic to see colors of green, and white, and not a green ground "screw" itself nor a silver one!

I also had to ask too ...:grin:
