manholes

Does NEC or industry practice require the following for manholes at 12,000 volt;

Water proofing on exterior being of grout or sealer
Drain in bottom with gravel
If more than one circuit to be identified with tags
Grounding of ladder, lid and all metal parts inside of the manhole

Any help in finding some of this info would be great!!

It looks like you are reading a pretty standard set of specs. I am not going to review the NEC right now but I can tell you in my experience the items you listed are very common in medium voltage man holes. You can usually add to that fire tapping of each circuit, minimum racking and supporting requirements, and in some cases sump pumps in each man hole that drain to the storm drain system.
