i was failed on an installation of recessed cans in a commercial soffit. inspector wants support of mc cable within 12" of the fixture box. the mc comes in the top of the box and has a small loop before being stappled to the joist. 18" to 20" of cable from the fixture. my question is how is the last sentence of nec 330.30 d applied to the section? is the cable secured within 12" of the box when an mc connector is used?

Is the ceiling accessible? If not then nothing in 330.30(D) would help you.

330.30(D) Unsupported Cables.
Type MC cable shall be permitted to be unsupported and unsecured where the cable complies with any of the following:
(1) Is fished between access points through concealed spaces in finished buildings or structures and supporting is impractical.
(2) Is not more than 1.8 m (6 ft) in length from the last point of cable support to the point of connection to luminaires or other electrical equipment and the cable and point of connection are within an accessible ceiling.
(3) Is Type MC of the interlocked armor type in lengths not exceeding 900 mm (3 ft) from the last point where it is securely fastened and is used to connect equipment where flexibility is necessary to minimize the transmission of vibration from equipment or to provide flexibility for equipment that requires movement after installation. For the purpose of this section, Type MC cable fittings shall be permitted as a means of cable support.

any suggestions on how to support the cable in the middle of 2ft ceiling joist? if you enter the top of the fixture the cable is more than 12" by the time it gets to the joist.

330.30d3 last Scentence. for the purpose of this section, type mc cable fittings shall be permitted as a means of cable support. where / how does this apply to this section?

Note that 330.30(B) requires securement, and the sentence your are citing only talks about support. Those are two different things. Your installation required securement and an installation above an accessible ceiling only requires support.
