Mc sleeve

Does mc need to be sleeved through 4" inch concreat slab ?
I thought bc concreat contains moisture so mc should be transfered to conduit when penetrated concreat slab to panel.

Is it through a hole in the concrete or embedded in the concrete?

The mc feeder is from the 1st floor it penetrates a 4" slab and it enters the bottom of a panel.
